Independent Contractor Agreement

This agreement is to be used for the services to be performed by an individual,. It is not to be used conjunction with any other company, corporation or other business association. No work shall be performed until this contract is fully completed and approved by all parties. Any work performed prior to such date shall be stated and included in this agreement.
